If you recieved an email about using our EZ-Check in for an upcoming appt or an email saying your have documents to review, you WILL NOT NEED a token, as you are already active on our portal. Please log in using your email as your username. If you cannot remember you password please call the office and we will be happy to reset it for you. OR text RESET PORTAL PASSWORD to 237-5510.
